What is 1040-V?

Form 1040-V, Payment Voucher, is a statement that accompanies a check or money order paying the amount you owe on your individual income tax return. It carries your name, SSN, address, and payment amount so the IRS can credit the payment to the right account even if it gets separated from your return.

You only need it when paying by mail. If you pay electronically through IRS Direct Pay, EFTPS, or a card processor, no voucher is required.

Who files 1040-V?

  • Individuals mailing a check or money order for the balance due on Form 1040 or 1040-SR
  • Nonresident filers paying a balance due on Form 1040-NR by mail
  • Taxpayers who filed electronically but chose to send their payment by mail
  • Anyone paying online does not need this form at all

How to fill out 1040-V

  1. Enter your SSN, and your spouse’s SSN if you filed jointly, in the same order as on your return.
  2. Write the amount you are paying exactly as it will appear on your check or money order.
  3. Fill in your name and address as shown on your return.
  4. Make the check or money order payable to United States Treasury and write your SSN, the tax year, and the form number on it.
  5. Download and print the voucher, then mail it with your payment (do not staple them together) to the address for your state in the 1040-V instructions.

No. The IRS asks that you not staple or attach the payment to the voucher or the return. Place them loose in the same envelope.
