What is 1195?

The Identity Declaration works in a similar way to the identity check on a passport application. A recent photograph of the applicant is endorsed by an approved person who has known the applicant for a required period, confirming that the photograph is a true likeness and that the personal details are correct. This helps the Department of Home Affairs establish who the applicant is.

Form 1195 sets out the applicant details, the details of the person endorsing the declaration, and the declaration itself. The endorser must belong to an accepted occupation or category and must meet the length of acquaintance stated on the form, so it is worth checking those requirements before asking someone to complete it.

How to fill out 1195

Enter the applicant personal details as they appear on the visa application.
Attach a recent photograph of the applicant that meets the size and quality requirements on the form.
Have an approved endorser, who has known the applicant for the required period, complete their section and endorse the photograph.
Check that the endorser occupation or category is one the form accepts.
Sign the declaration and lodge the completed form with the Department of Home Affairs as part of the visa application.
