What is 3911?
Form 3911, Taxpayer Statement Regarding Refund, tells the IRS a refund went missing so it can trace the payment. For a check, the Bureau of the Fiscal Service checks whether it was cashed and can send a claim package for a forged endorsement; uncashed checks are cancelled and reissued. For direct deposits, the trace follows the deposit to the receiving bank.
Before filing, allow the normal windows: 28 days from the check’s mailing date, and confirm the refund status in "Where’s My Refund" first. Married-filing-jointly refunds usually require both spouses’ signatures on the trace.

What if the check was cashed by someone else?
