What is 5498?

Form 5498, IRA Contribution Information, is filed by the financial institution that acts as trustee or custodian of an individual retirement arrangement. A separate form is prepared for each person for whom an IRA is maintained, including deemed IRAs under section 408(q), and it reports contributions, rollover amounts, Roth conversions, recharacterizations, required minimum distribution details, and the fair market value of the account.

Account owners do not file Form 5498 with their tax returns. It is an informational copy that helps them confirm what the custodian reported to the IRS, which matters when claiming an IRA deduction or tracking basis in nondeductible or Roth contributions.

Who files 5498?

  • Banks, brokerages, and other custodians that maintain traditional, Roth, SEP, or SIMPLE IRAs
  • Trustees reporting rollovers, conversions, or recharacterizations that occurred during the year
  • Issuers reporting the December 31 fair market value and any RMD information for each account
  • Custodians of deemed IRAs under section 408(q) within employer plans

How to fill out 5498

  1. Enter the trustee or issuer’s name, address, and TIN, along with the participant’s identifying information.
  2. Report regular contributions in box 1 and rollover, conversion, or recharacterization amounts in their own boxes.
  3. Mark the IRA type checkboxes and enter the year-end fair market value and any RMD details the instructions call for.
  4. Download the completed PDF and give the participant their copy by the deadline in the current general instructions.
  5. File Copy A with the IRS electronically through IRIS or FIRE, or on official scannable paper stock if you qualify for paper filing.
Worth knowing: Copy A of Form 5498 filed with the IRS must be e-filed through IRIS or FIRE, or submitted on official scannable red-ink forms ordered from the IRS. Do not print and mail Copy A from a downloaded PDF; it is not scannable and can trigger penalties.
