What is DA Form 4036?
DA Form 4036, Medical and Dental Preparation for Overseas Movement, is a two-page Army checklist that documents a Soldier's medical and dental readiness before overseas movement. Medical and dental staff work through items such as immunizations, physical exams, dental classification, medications, eyeglasses and protective mask inserts, and any profiles or conditions that could limit deployment.
The form is completed during readiness processing rather than by the Soldier alone. Each section is verified and signed off by the clinic or SRP site, and the finished form becomes part of the movement packet that shows the Soldier is cleared to go.
