What is Form 13?
The Notice of intention to leave (Form 13) is an approved form under the Residential Tenancies and Rooming Accommodation Act 2008, issued by the Residential Tenancies Authority (RTA). A tenant uses it to tell the property manager or owner, in writing, the date they will vacate and the reason the tenancy is ending.
The two page form records the rental property address, the tenant and lessor or agent details, the handover date, and the ground for leaving. Grounds include ending a periodic agreement, reaching the end of a fixed term, an unremedied breach by the lessor, the premises being unliveable, or the property being sold. The required notice period depends on the ground you select, so read the notes on the form before you set the handover date.
