What is SF 3107?

SF 3107, Application for Immediate Retirement under the Federal Employees Retirement System, is the core form of a FERS retirement package. It collects your federal service history, military service and deposit information, survivor annuity elections, insurance continuation information, and payment instructions. Married applicants electing less than a full survivor annuity need spousal consent on SF 3107-2, which is part of the package.

The form is long because your elections here are consequential: the survivor benefit you choose affects your spouse's lifetime income, and unpaid deposits for military or temporary service can change your annuity. Your agency completes SF 3107-1, the Certified Summary of Federal Service, and forwards the whole package to OPM after your separation is processed.

How to fill out SF 3107

Confirm your eligibility and planned retirement date with your agency HR or benefits office before you start.
Complete the identification, federal service, and military service sections, noting any deposits or redeposits you have made or owe.
Make your survivor annuity election; if you are married and elect less than the maximum, your spouse must consent on SF 3107-2 before a notary.
Answer the insurance and other-benefit questions, attach any required documentation such as your marriage certificate or military discharge papers, and sign the application.
Submit the package to your employing agency's HR office, not to OPM. The agency certifies your service and payroll records and forwards everything to OPM, which finalizes your annuity.

Most agencies suggest submitting your application well before your retirement date so HR can assemble your records; many recommend at least 60 days. Interim payments from OPM begin after your agency transfers the package, and full adjudication can take additional months.

What if I separated from federal service more than 30 days ago?

SF 3107 is for immediate retirement. If you left federal service earlier and are applying for a deferred annuity, a different application applies (RI 92-19 for FERS deferred retirement). Check OPM's retirement services pages for the right form for your situation.

Can I change my survivor election after I retire?

Only in limited circumstances and within strict time limits set by law, such as shortly after retirement or after a marriage. Treat the election on SF 3107 as final and confirm current rules with OPM before assuming a change is possible.
