4. Odometer disclosure

Federal law requires the seller to state the mileage upon transfer of ownership for most vehicles. The seller certifies that the reading below reflects the actual mileage unless a box is checked.

6. Signatures

Both parties should keep a signed copy, and the buyer will usually need it for title transfer and registration. Sign in ink after printing. Many uses of this document require witnesses or a notary; check the rules that apply in your state before signing.
