What is Form 12?
The Notice to leave is issued under the Residential Tenancies and Rooming Accommodation Act by a lessor or agent to ask a tenant to vacate a rental property. It states the ground for ending the tenancy, such as the end of a fixed term agreement, a periodic agreement ending, unremedied breach, or the property being unfit to live in, and it sets a handover day by which the tenant is asked to leave.
The minimum notice period depends on the ground given. A standard end of a fixed term agreement generally requires two months, while grounds such as the premises being unlivable can take effect the day the notice is given. When the notice is served by post, extra time must be allowed for delivery when working out the last day of the notice period. The form applies to general tenancies and moveable dwelling agreements in Queensland.

- Property managers or agents serving notice on a tenant on behalf of a lessor
- Owners ending a fixed term or periodic agreement, or acting on an approved ground

How to fill out Form 12
- Enter the rental property address and the names of the lessor or agent and the tenant.
- Select the ground for the notice, such as end of a fixed term agreement or unremedied breach.
- Set the handover day, allowing the minimum notice period for the ground you have chosen.
- If serving by post, allow extra time for delivery when working out the last day of the period.
- Sign and date the notice and give it to the tenant, keeping a copy and a record of how it was served.
