What is 4506-T?
Form 4506-T requests any of five transcript types: return transcript (most lines of your filed return), account transcript (payments, penalties, adjustments), record of account (both combined), wage and income transcript (your W-2s and 1099s as reported to the IRS), and verification of non-filing. Transcripts are free, unlike full return copies which cost money via Form 4506.
Most individuals can skip the form entirely with the IRS online transcript service; the paper request matters when a third party needs it mailed, for older years, or when online identity verification fails.

How to fill out 4506-T
- Enter your name and SSN as they appeared on the return, with a spouse for joint years.
- Give your current address and, if the IRS has an older one on file, the previous address.
- Check the transcript type you need and list the tax years.
- Download, sign within 120 days of the date you will submit it, and mail or fax it to the IRS location listed for your state.
