The same form is used to revoke an earlier election and start contributing again.

Who can use CPT30?
An employee who is at least 65 but under 70 and is receiving a CPP or QPP retirement pension can use CPT30 to stop contributing. If you are self-employed only, a different election applies through your tax return.
Do I give this form to my employer or the CRA?
Both. You give a copy to each employer so they stop deducting CPP, and you send the original completed form to the Canada Revenue Agency.
