What is DD Form 2864?
DD Form 2864, Voluntary Separation Incentive (VSI) Designation of Beneficiary, is used by former service members who separated under the VSI program, which paid an annual incentive over a period of years to members who left voluntarily during the post-Cold War drawdown. Because VSI payments continue for a set number of years, the program needs to know who should receive any payments still owed when a recipient dies.
The form records the recipient’s identifying information and the designated beneficiary or beneficiaries, with shares for each. Keeping the designation current matters most after life events such as marriage, divorce, or a death in the family, since payments follow the designation on file.
