What is SF 1174?

SF 1174, Claim for Unpaid Compensation of Deceased Member of the Uniformed Services, is used to claim final pay and allowances due a service member who died: unpaid basic pay, accrued leave, travel allowances, and similar amounts. It is separate from life insurance (SGLI) and from the death gratuity, which are paid under their own procedures.

Unpaid compensation is paid in a set order of precedence: to the beneficiary the member designated, and otherwise to survivors in the order fixed by law, generally starting with the surviving spouse and then children. The form collects the claimant's relationship to the deceased and information about other potential survivors so the payment goes to the right person.

How to fill out SF 1174

Fill in the deceased member's name, service, and identifying information, along with the date of death.
Complete the claimant section, stating your relationship to the member and listing other surviving family members as the form requires.
Attach the documentation requested, such as a certified copy of the death certificate, and appointment papers if you are claiming for an estate.
Sign the claim before a witness as the form directs.
Submit the claim as instructed by your casualty assistance officer, typically to DFAS or the relevant service's military pay office. Do not send it to GSA.

Frequently asked questions

Is SF 1174 the same as claiming SGLI life insurance?

No. SGLI is claimed separately through the insurance program. SF 1174 covers only unpaid compensation, such as final pay and unused leave, that the service member had earned before death.
