Additional Family Information, IMM 5406, is issued by Immigration, Refugees and Citizenship Canada (IRCC). It is a supporting form used across many permanent residence and some temporary applications to list an applicant’s family members. It captures your spouse or partner, all of your children, your parents, and your siblings, including their dates of birth, addresses and occupations, and whether each one is accompanying you.
