What is IMM 5645?
The Family Information Form gathers a full picture of an applicant immediate family. It asks for the name, date and place of birth, current address, marital status, present occupation and relationship of the applicant spouse or partner, all children, both parents, and every brother and sister, whether or not they are coming to Canada. It is a standard piece of family class and permanent residence packages.

Do I include family members who are not immigrating?
Yes. IMM 5645 must list all immediate family whether or not they are coming to Canada, including a spouse, all children, both parents and every sibling. Leaving people off can cause serious problems later.
What if a family member has died?
Still list them and mark the entry as deceased, giving the date where the form asks. The form is meant to show your complete family, not only living members.
