What is IMM 5475?
The Authority to Release Personal Information to a Designated Individual, IMM 5475, is issued by Immigration, Refugees and Citizenship Canada (IRCC). It lets you give a named person permission to obtain information about your application, for example so a family member can call and ask about its status. It is not a representative form: the designated individual can receive information but cannot conduct business or make decisions on your behalf.

Is IMM 5475 the same as appointing a representative?
No. IMM 5475 only lets a named person receive information about your case. To let someone conduct business or act for you with IRCC, you use the Use of a Representative form, IMM 5476, instead.
How do I cancel the authority?
You can cancel it by notifying IRCC in writing, and a new authorisation naming a different person does not automatically override an earlier one. You may also set an end date when you complete the form.
Why does the form open blank in my browser?
