IMM 5707: Family Information Form for Visitors, Students and Workers

What is IMM 5707?

This form is the temporary residence counterpart to the family class Family Information Form. It captures the applicant spouse or common-law partner, all children, both parents and every sibling, with dates of birth, addresses, marital status and occupations. It accompanies visitor visa, study permit and work permit applications so officers can see the applicant family ties.

IMM 5707 is a dynamic XFA form published by Immigration, Refugees and Citizenship Canada. It does not render in a browser tab and an online editor shows it blank. Download it and open it in Adobe Acrobat Reader on a desktop or laptop to enter each family member details, then validate the form before submitting it to IRCC.

How to fill out IMM 5707

1. Download IMM 5707 and open it in Adobe Acrobat Reader rather than a browser.
2. Enter your spouse or common-law partner details, then each of your children.
3. Add both parents with their names, dates of birth and current addresses.
4. List every brother and sister, including any who are deceased.
5. Validate, sign and date the form, then submit it with your application to IRCC.
