What is LH 0082?
Your employer or benefits agency withholds loonbelasting and premie volksverzekeringen from your pay, and to do that correctly it needs your personal details. This model form is what the Belastingdienst publishes for collecting them.
The form also asks whether you want the loonheffingskorting applied through this employer. You can only have it applied by one at a time, so the answer matters when you have two jobs or a job alongside a benefit.

- Check whether your employer has already filled in part of the form before you add anything.
- Enter your surname, initials, address, date of birth and burgerservicenummer.
- Say whether the loonheffingskorting should be applied, and from which date.
- Sign and date the form, and hand it to your employer together with proof of identity.
- Your employer keeps the form in the payroll records; it is not sent to the Belastingdienst.

What happens if I do not hand this in?
The form is blunt about it: without your details, or with wrong ones, your employer must withhold 52 percent payroll tax. It also affects whether you can be identified correctly.
Can two employers apply the payroll tax credit?
No, only one at a time. Applying it twice means too little tax is withheld and you get a bill later.
Do I send this to the tax office?
No. It stays with your employer, in the payroll administration.
