What is DV 1601?
The form covers an objection to a final or additional assessment for income tax and national insurance contributions, to an assessment for the income-related healthcare contribution, and to the income figure used for a toeslag, a personal contribution or an allowance.
It asks you to say what you disagree with and why, and to give the figure you think is right. That reasoning is the substance of the objection, so it is worth writing it out properly rather than in one line.

How to fill out DV 1601
- Keep the assessment in front of you: you need its date, its number and the amount.
- Fill in your own details, including your burgerservicenummer.
- Identify the assessment you are objecting to and state clearly what should change.
- Explain why, and give the amount you believe is correct.
- Sign the form and post it to Belastingdienst, Postbus 2572, 6401 DB Heerlen.

Frequently asked questions
How long do I have?
Six weeks from the date on the assessment. An objection sent after that is normally treated as a late request rather than a valid objection.
Can I object to a provisional assessment?
No. The form says so directly: a voorlopige aanslag cannot be objected to, only changed or stopped.
I have two assessments to contest.
Use a separate copy of the form for each one. The Belastingdienst asks for one objection per assessment.
