What is SF 1199A?
SF 1199A, Direct Deposit Sign-Up Form, authorizes a federal agency to send your recurring payments electronically to an account at your bank or credit union. It is used across many payment types, including Social Security and SSI benefits, VA compensation and pension, civil service annuities, and military pay and allowances.
The form has three sections: the payee completes the first with personal and payment information, the paying agency is identified in the second, and the financial institution completes and certifies the third with the routing and account details. Because the bank must certify its section, the form usually gets finished at a branch or through the institution before it goes to the government agency that pays you.
Who files SF 1199A?
- Social Security, SSI, and other federal benefit recipients setting up or changing direct deposit
- Veterans receiving VA compensation, pension, or education payments
- Federal annuitants and survivor annuitants paid by OPM

How to fill out SF 1199A
- Complete Section 1 with your name, address, identifying information, the type of payment, and the account type.
- Take the form to your bank or credit union so it can complete and certify Section 3 with your routing and account numbers.
- Confirm the paying agency information in Section 2 matches the agency that actually sends your payment.
- Sign the payee certification.
- Send the completed form to the government agency that issues the payment, not to GSA or the Treasury. For Social Security payments you can often skip the paper form entirely by enrolling through your bank, by phone, or online.

Where do I send the completed SF 1199A?

Do I need this form to switch bank accounts?
You need to enroll the new account before closing the old one so no payment is missed. Many agencies now offer online or phone enrollment that replaces the paper form; SF 1199A remains the paper route and is still accepted.
Why does my bank have to sign it?
Section 3 is a certification by the financial institution that the account details are correct and that it will accept the electronic payments. Agencies will not process the form without it.
