Form SF100 is the claim for the Sure Start Maternity Grant, a one off payment of £500 from the Department for Work and Pensions towards the cost of a new baby in England and Wales.

What is SF100?

The Sure Start Maternity Grant is a £500 payment that does not have to be repaid and does not affect your other benefits. You normally qualify if you are expecting your first child, or expecting a multiple birth such as twins when you already have children, and you or your partner receive a qualifying benefit or tax credit at the right rate.

The claim must be made within the window DWP sets: from 11 weeks before the week the baby is due until 6 months after the birth. Part of the form is completed by a health professional, who either fills in a MAT B1 maternity certificate or writes a statement confirming the pregnancy or birth. Claims in Scotland go to Social Security Scotland for the Pregnancy and Baby Payment instead.

People becoming responsible for a child through adoption, a parental order or a surrogacy arrangement

How to fill out SF100

Check you are within the claim window: 11 weeks before the due date up to 6 months after the birth.
Complete your own details, your partner's details if you have one, and the benefit you or your partner receive.
Give the baby's details or expected date of birth, and answer the questions about other children in your household.
Ask a doctor or midwife to complete the health professional section, or attach your MAT B1 or a signed statement from them.
Sign the declaration and post the completed form with any supporting evidence to Freepost DWP SSMG. No stamp or postcode is needed.

Worth knowing: The form is not valid without the health professional part, so allow time for a midwife or doctor to complete it or to give you a MAT B1 before the 6 month deadline passes.

Frequently asked questions

How much is the Sure Start Maternity Grant?

The grant is £500 for a first child, or for a multiple birth where you already have children. It is a one off payment that you do not repay and it does not reduce your other benefits.

What if I miss the deadline?

The claim has to reach DWP within the window that runs from 11 weeks before the due date to 6 months after the birth. Claims made outside that period are normally refused, so send it as early as you can.

I live in Scotland, can I use SF100?

No. Scotland has its own scheme, the Pregnancy and Baby Payment from Social Security Scotland. SF100 covers England and Wales, and Northern Ireland has separate arrangements.
