What is SS-5?
Form SS-5 applies for an original Social Security number, a replacement card, or a correction to the record, most commonly a name change after marriage. The completed form goes to the Social Security Administration with original or certified evidence documents: proof of identity always, plus citizenship or immigration status and age for first-time cards.
Many replacements can now be handled through an SSA online account without the paper form; the SS-5 remains the route for first cards, name changes, and anyone who cannot use the online service.
Who files SS-5?
- Parents applying for a child’s first number, when not done at birth registration
- Adults who changed their name and need the card corrected
- People replacing a lost or stolen card
- New arrivals with work authorization applying for a first number
How to fill out SS-5
- Fill in the name to appear on the card and any prior names used.
- Enter birth details, citizenship or immigration status, and parents’ information.
- Answer whether you have had a number or card before.
- Download and sign, then take or mail it to your Social Security office with original or certified evidence documents, which the SSA returns.

Which documents prove identity?
