What is SSA-89?

SSA-89, Authorization for the Social Security Administration To Release Social Security Number Verification, is the written consent used with SSA's Consent Based Social Security Number Verification (CBSV) service. You sign the form for a specific company, and that company, or its agent enrolled with SSA, submits your name, SSN, and date of birth to SSA, which answers whether the combination matches its records.

The verification is a yes-or-no match. It does not disclose your earnings, benefits, or other file information, and it does not prove identity or immigration status by itself. The form names the company receiving the verification and the reason for the request, and the consent is valid only for a limited period stated on the form, so it cannot be reused indefinitely or by a different company.

Who files SSA-89?

  • Mortgage and loan applicants whose lender requires SSN verification as part of underwriting or fraud prevention
  • People opening financial accounts where the institution uses CBSV checks

How to fill out SSA-89

  1. Print your name, Social Security number, date of birth, and address exactly as they appear in SSA records.
  2. Confirm the name of the company (and its agent, if any) that will receive the verification and the stated reason for the request; the requesting business usually fills these in.
  3. Read the consent language so you understand what SSA will confirm and for whom.
  4. Sign and date the form; consent is valid only for the period stated on the form, so sign close to when the company will submit it.
  5. Return the signed form to the requesting company, which submits it through SSA's CBSV service; it is not mailed to SSA by you.

Why is my lender asking for SSA-89?

Lenders use SSN verification to guard against identity theft and synthetic identity fraud before funding a loan. The check simply confirms with SSA that the name, SSN, and date of birth you provided match its records; it does not pull your credit or reveal benefit information.

Does SSA-89 give the company access to my Social Security file?

No. The response is a match or no-match on the identifying information, plus an indication if SSA records show the number holder is deceased. Earnings history, benefit amounts, and other file contents are not released under this consent.

How long is the signed form good for?

The consent is valid only for the limited period stated on the form itself, measured from the date you sign, unless you write in a shorter period. If the company does not submit it in time, you will need to sign a fresh form, so check the current version at ssa.gov for the exact window.
