What is T2201?
T2201, the Disability Tax Credit Certificate, is the form the CRA uses to decide whether a person qualifies for the Disability Tax Credit (DTC). The applicant fills in Part A with their identifying details, and a recognised medical practitioner completes Part B to certify the nature and duration of the impairment. Once the CRA approves the certificate, the person, or a supporting family member, can claim the credit on their income tax and benefit return, and being approved can also open the door to related programs such as the Registered Disability Savings Plan.
T2201 is a dynamic PDF that the CRA publishes in Adobe XFA format. It does not fill in a web browser, so it will not display in the in-browser editor or quick-fill form here. Download it and open it in Adobe Acrobat Reader to complete the fields, then have the medical practitioner certify their part before you submit it.

Do I need to send T2201 every year?
No. Once the CRA approves the certificate it stays valid for the period noted in the determination. You only reapply when that period ends or if the CRA asks you to update it.
