What is W-12?

Form W-12, IRS Paid Preparer Tax Identification Number (PTIN) Application and Renewal, is how a tax return preparer applies for a PTIN or renews an existing one by mail. Every person who prepares or substantially helps prepare federal tax returns for compensation must have a valid PTIN and enter it on the returns they sign.

Most preparers use the IRS online PTIN system instead because it is much faster; the paper W-12 can take several weeks to process. PTINs are issued for a calendar year and must be renewed annually, and a user fee applies. Preparers without a Social Security number may need to attach Form 8945 or 8946.

  • First-time paid tax return preparers applying for a PTIN by mail
  • Preparers renewing an existing PTIN for the coming filing season
  • Enrolled agents, CPAs, and attorneys who prepare federal returns for compensation

How to fill out W-12

  1. Enter your legal name, any prior names, your SSN, and your date of birth, and mark whether this is an initial application or a renewal.
  2. Provide your mailing address, business address, phone, and email, plus your business website if you have one.
  3. Answer the questions about felony convictions, federal tax compliance, and your professional credentials.
  4. Sign and date the form, and attach Form 8945 or 8946 if you have no SSN.
  5. Download the finished PDF and mail it with the current user fee to the IRS address in the instructions. Or skip the mail entirely: the online PTIN system at irs.gov issues most PTINs the same day.

How long does a paper W-12 take versus applying online?

The paper form can take four to six weeks to process, while the online PTIN system usually issues or renews a PTIN in about 15 minutes. Use the paper form only if you cannot apply online.

Does a PTIN expire?
