Form 1022: Notification of Changes in Circumstances

What is 1022?

Under the Migration Act, visa applicants must tell the Department of Home Affairs about relevant changes in their circumstances before a decision is made. Form 1022 is the standard way to do this. It covers changes such as a new address, a new passport, a change in relationship status, the birth of a child, or other facts that affect an application already on file.

The form asks you to identify the application affected, describe what has changed, and give the new details. Keeping the department up to date matters because a decision made on out of date information can be affected, and failing to disclose a relevant change can raise character or fraud concerns against the application.

Who files 1022?

• Visa applicants whose address, passport or contact details have changed
• Applicants whose relationship status, family composition or dependants have changed
• People who need to correct or update information already given to Home Affairs
• Anyone directed by the department to notify a change using form 1022

How to fill out 1022

1. Identify the visa application affected, including the application or reference number if you have one.
2. Enter your personal details as they appear on the original application.
3. Describe clearly what has changed and give the new details, such as a new address or passport number.
4. Attach any supporting evidence for the change, for example a new passport bio page or a birth certificate.
5. Sign the declaration and lodge the completed form with the Department of Home Affairs.
