What is Form 15?
Form 15, Abandonment Termination Notice, is issued by the lessor or their agent under Queensland residential tenancy law when there are reasonable grounds to believe a rented property has been abandoned. It sets out the details of the tenancy and states that the agreement is being terminated on abandonment grounds.

How to fill out Form 15

- Set the date the notice takes effect in line with Queensland tenancy rules.
- Sign and date the notice as the lessor or agent.
- Serve the completed notice on the tenant and keep a copy with your tenancy records, in line with Residential Tenancies Authority requirements.

Can a tenant dispute an abandonment notice?
Yes. A tenant can apply to QCAT to dispute the notice within a short window after it is served. If they do not dispute it, they are generally treated as having abandoned the premises.
Do I have to confirm the property is abandoned first?
You must have reasonable grounds for believing the premises has been abandoned. Some managers issue an entry notice first to check. Follow the process set out by the Residential Tenancies Authority.
