What is 4506?
Form 4506, Request for Copy of Tax Return, is how you order a complete copy of a previously filed return, with the schedules and attachments as filed. You can also use it to have the copy sent to a third party you designate. The IRS charges a fee for each return requested; check the current form for the amount.

- Download the PDF, sign and date it, and mail it with your check or money order to the IRS address listed in the form’s chart for your state.
