What is 4506-C?
Form 4506-C, IVES Request for Transcript of Tax Return, lets an IVES participant, typically a mortgage lender or its vendor, obtain your tax transcripts directly from the IRS with your written consent. Lenders use it to verify the income you reported when underwriting mortgages, business loans, and other credit.
Unlike Form 4506, it delivers transcripts (summaries of your return data), not photocopies, and it is processed through the IVES program rather than mailed to you. The lender usually fills in its own IVES participant information and asks you to complete the taxpayer sections and sign.
Who files 4506-C?
- Borrowers applying for a mortgage, refinance, or business loan whose lender needs IRS income verification
- Self-employed applicants whose tax returns are the main proof of income
- Co-borrowers and spouses whose transcripts the lender also needs, each signing where required
- Lenders and IVES participants preparing the form for a customer’s signature
How to fill out 4506-C
- Check the lender’s sections first: the IVES participant name and information are usually pre-filled by the lender.
- Enter your name, SSN or EIN, and current address exactly as they appear on the tax return being verified.
- Confirm the transcript type and tax years requested, which the lender typically specifies.
- Sign and date the form; a spouse must also sign if joint transcripts are requested and the form requires it.
- Download the PDF and return it to your lender. It goes to the IRS through the IVES system, not by mailing it yourself.

Frequently asked questions
What is the difference between Form 4506-C and 4506-T?
Both request transcripts, but the 4506-C is exclusively for the IVES program, where an approved participant such as a lender receives the transcript. The 4506-T is the general transcript request form.
Is it safe to sign a 4506-C for my lender?
The form only authorizes the transcripts, years, and recipient listed on it. Read those lines before signing and make sure the request matches what you agreed to in your loan application.
Do I send Form 4506-C to the IRS myself?
No. Your lender submits it through its IVES channel. If you want transcripts for your own use, request them free at irs.gov or with Form 4506-T instead.
