DA Form 1058, Application for Active Duty for Training, Active Duty for Operational Support, and Annual Training for Soldiers of the Army National Guard and U.S. Army Reserve, is the application a Reserve Component soldier submits to request a voluntary period of active duty. It is prescribed under AR 135-200 and covers the tour type, duty dates, duty location, and funding information for the requested period.
