What is DA Form 1380?

DA Form 1380, Record of Individual Performance of Reserve Duty Training, documents individual training duty performed by Reserve Component soldiers so it can be certified and credited. It is commonly used to claim retirement point credit for duty performed away from the soldier's unit or outside regularly scheduled assemblies.

The current edition on Army Publishing Directorate is dated April 2025, with the Army G-1 as proponent. The two page form records the soldier's identification, the dates and description of the duty performed, the type of duty, and certification by an authorized official.

How to fill out DA Form 1380

Record the dates, location, and hours of the duty performed.
Describe the training or duty in the performance blocks, one entry per period of duty.
Sign the form and obtain certification from the authorized official who can verify the duty.
Submit the certified form to your unit administrator or personnel office so the duty is posted to your record.

Worth knowing: Keep your own copy of every certified DA Form 1380. If credited duty is ever questioned, the certified form is your proof that the training was performed.

When do I need DA Form 1380 instead of normal drill attendance records?

Regular unit assemblies are recorded through the unit's attendance system. DA Form 1380 is generally used when duty is performed individually, away from the unit, or in another status where attendance is not captured automatically, so the duty can still be certified and credited.

Who certifies my DA Form 1380?

An authorized official who can verify the duty was performed, typically the supervising officer or official at the place of duty. Uncertified forms cannot be credited, so get the certification before you leave the duty location when possible.

How do the retirement points get posted?

After certification, the form goes to your unit administrator or personnel office, which posts the duty to your record. Keep a copy and check your retirement points statement to confirm the credit appears.
