What is DA Form 5960?
DA Form 5960, Basic Allowance for Housing (BAH) Authorization and Dependency Declaration, certifies the facts that determine a Soldier's housing allowance: marital status, dependents, their addresses, and where the Soldier lives. Soldiers complete it at in-processing, after life events like marriage, divorce, or the birth of a child, and whenever their housing situation changes.
The form is a sworn declaration, so accuracy matters; supporting documents such as marriage or birth certificates are usually required with it. The current form is governed by AR 637-1, and finance offices audit BAH entitlements against it.
