What is DA Form 7279?

DA Form 7279, Equal Opportunity and Harassment Complaint Form, is used by Soldiers and, in covered situations, Army civilians and family members to report discrimination or harassment. The form records the complainant's information, a description of the incident with dates, locations, and people involved, the names of witnesses, and the resolution the complainant is seeking.

A complaint can be informal, resolved at the lowest possible level, or formal, which triggers a documented investigation with defined timelines under AR 600-20.
