What is DA Form 1594?

DA Form 1594, Daily Staff Journal or Duty Officer's Log, is a chronological record of events for a staff section, operations center, or duty position. Each line records the time an incident or message occurred, a summary of it, and the action taken, creating an official record of the period of duty.

The current edition on Army Publishing Directorate is dated December 2019. The single page form has header blocks identifying the organization, location, and period covered, followed by numbered item lines with time in, time out, incident or message text, and action taken, and closes with the signature of the officer or official on duty.

How to fill out DA Form 1594

  1. Fill in the organization, location, and the period covered by the journal in the header.
  2. Log each event on its own line with the time and a concise summary of the incident or message.
  3. Record the action taken for each entry, including who was notified.
  4. Continue entries chronologically for the entire duty period; do not leave gaps or erase entries.
  5. Sign the journal at the end of the period and turn it in as your unit's procedures direct.
