What is DA Form 7425?
DA Form 7425, Readiness and Deployment Checklist, is the Army Publishing Directorate form used at Soldier Readiness Program (SRP) events. It walks a Soldier and the processing stations through the items that must be verified before deployment and again at redeployment: personnel records, ID cards and tags, finance, legal documents such as wills and powers of attorney, medical and dental status, and required training.
Each section is completed and initialed by the responsible station (personnel, finance, legal, medical, and so on) rather than by the Soldier alone. The finished checklist becomes part of the deployment packet, so units can show that every readiness requirement was checked before the Soldier left and after the Soldier returned.

- Soldiers going through Soldier Readiness Processing before a deployment or mobilization
- Soldiers completing redeployment processing after returning from a theater of operations
- Unit readiness NCOs and S1 staff preparing personnel for an SRP event
- Installation SRP site staff who verify and initial each processing station
How to fill out DA Form 7425
- Fill in the Soldier identification block: name, rank, unit, and the processing date.
- Work through the personnel and finance sections, confirming records, ID card, dependents, and pay data are current.
- Complete the legal, medical, and dental blocks as each station reviews the Soldier and initials its portion.
- Record required training and any items that need follow-up before the Soldier can be validated.
- Download the completed PDF and turn it in to the SRP site or your unit S1, who file it with the deployment packet.

Can I complete DA Form 7425 by myself?
You can fill in your identifying information and the parts you know, but most blocks are verified and initialed by the SRP processing stations. Bring the form to the event rather than treating it as a self-certification.
