What is DA Form 7708?
DA Form 7708, Personnel Reliability Screening and Evaluation, is the Army form a certifying official uses to decide whether a person is reliable enough for sensitive duties, most commonly positions involving access to arms, ammunition, and explosives. The individual answers screening questions about their record and circumstances, and consents to the records checks the evaluation requires.
The certifying official then reviews the answers along with available personnel, medical, and law enforcement information and records a determination on the form. The proponent is the Office of the Provost Marshal General, and the current edition was published in 2019.
