What is DS-64?
Form DS-64 is the State Department's statement for reporting a lost or stolen passport book or passport card. Once the report is processed, the passport is invalidated and can no longer be used for international travel, even if you later find it.
You can report a lost or stolen passport online through travel.state.gov, by phone, or by submitting this form. If you are applying for a replacement at the same time, DS-64 is typically submitted together with Form DS-11 in person at a passport acceptance facility.
