What is DS-82?
Form DS-82 is the mail-in renewal. Eligibility is specific: you have the passport, it is undamaged, it was issued when you were 16 or older, within the last 15 years, and in your current name or one you can document changing. The old passport is mailed in with the application and returned separately after the new one is issued.
Name changes ride along with a certified marriage certificate or court order. Applicants abroad and those failing any condition use the DS-11 in person instead. An online renewal option also exists for many applicants at travel.state.gov.
