What is DS-11?

Form DS-11 applies for a US passport book or card through an in-person appearance. It gathers identity, citizenship, parentage, and travel plans, and pairs with citizenship evidence (a birth certificate or naturalization certificate), a photo ID, photocopies of both, one passport photo, and the State Department and acceptance fees.

Applicants appear at an acceptance facility, most commonly a post office, or a passport agency for urgent travel. Children under 16 appear with both parents or documented consent. Renewals by eligible adults use the mail-in DS-82 instead.

Anyone whose previous passport was lost, stolen, or issued more than 15 years ago

How to fill out DS-11

Complete every field, including your Social Security number, travel plans, and emergency contact.
Print the form single-sided; do not sign it yet.
Gather citizenship evidence, photo ID, photocopies, and one compliant photo.
Appear at an acceptance facility, swear the oath, and sign only when the agent instructs.
Pay the application fee to the Department of State and the execution fee to the facility.

Frequently asked questions

Why can’t I sign in advance?

The DS-11 signature must be witnessed by the acceptance agent administering the oath. A pre-signed form is rejected on the spot.
