What is Form A2?
Form A2 records the full medical examination that a migrant domestic worker must undergo before her Work Permit is issued. It carries her personal particulars, a medical history she declares and signs herself, and the clinical findings and test results entered by the doctor. MOM uses it to confirm she is fit for work and free of the infectious diseases the screening covers.
The examination screens for tuberculosis, HIV, syphilis and malaria, and includes a general fitness assessment. Every part of the form has to be completed by a Singapore-registered doctor, any amendment has to be endorsed by that same doctor, and the helper’s travel document must be produced at the appointment so the doctor can confirm her identity.

How to fill out Form A2
- Book an appointment with a Singapore-registered doctor. MOM expects the examination to be done within two weeks of your helper arriving in Singapore.
- Print Form A2 and send your helper to the appointment with her passport or other travel document, which the doctor needs for identification.
- Your helper completes and signs the medical history in Part II, declaring conditions such as tuberculosis, epilepsy, diabetes and past operations.
- The doctor completes the clinical examination and test sections, signs, and stamps the form. Any correction must be endorsed by the same doctor.
- The completed results go to the Ministry of Manpower so the Work Permit can be issued. Ask the clinic for a copy for your own records, and check the current submission route on mom.gov.sg before the appointment.

What happens if my helper fails the medical examination?
MOM will not issue the Work Permit. Where a worker does not pass, she has to be sent home. Employers are generally responsible for the repatriation costs, so it is worth reading MOM’s guidance on employer obligations before she travels.
Does the examination have to be done in Singapore?
Yes. The form must be completed by a Singapore-registered doctor, and the chest X-ray is taken in Singapore. A medical report done overseas does not replace it.
Is this the same as the six-monthly medical examination?
No. Form A2 is the full examination done before the Work Permit is issued. Migrant domestic workers also need six-monthly medical examinations throughout their employment, including when they transfer to a new employer.
