What is Form B11?
Form B11 mirrors the examination done at first issuance, but is used when an existing migrant worker’s Work Permit is being renewed. It carries the worker’s personal particulars, a medical history the worker declares and signs, and the doctor’s clinical findings and test results.
As with the issuance form, all parts are completed by a Singapore-registered doctor, the worker’s travel document must be produced at the appointment for identification, and any amendment has to be endorsed by the doctor who completed the form. The worker also consents on the form to a copy being released to the Ministry of Manpower, the employer and the employment agent who assisted with the application.

How to fill out Form B11
- Book the examination with a Singapore-registered doctor in good time before the current Work Permit expires.
- Print Form B11 and send the worker to the appointment with their travel document.
- The worker completes and signs the medical history section, then consents to the results being released to MOM, the employer and the agent.
- The doctor completes the clinical examinations and tests, signs and stamps the form, and endorses any correction.
- Submit the completed results to the Ministry of Manpower as directed on the form. Check mom.gov.sg for the current submission route before the appointment, as MOM updates it from time to time.
