FDW Sponsorship Application: apply for a Foreign Domestic Worker under the Sponsorship Scheme

What is FDW Sponsorship Application?
MOM assesses a helper application largely on the employer’s income. Employers with low or no income can apply under the sponsorship scheme, where a family member such as a child, parent or sibling sponsors the application, or under the joint income scheme, where incomes are combined. Under these schemes the employer, the sponsor and any joint applicant must all be Singapore citizens or permanent residents.
The form runs to 18 pages and covers the helper’s particulars and job scope, the employer and the household, the sponsor or joint applicant, and the declarations MOM requires. You fill it in as a soft copy, then upload it with your supporting documents and pay the application fee through MOM’s online form. Income is proved with documents such as IRAS Notices of Assessment or CPF statements, or by giving MOM consent to verify income directly with IRAS.
Who files FDW Sponsorship Application?
- Employers applying for a helper with the support of a sponsor who is a close family member
- Employers applying under the joint income scheme, where a spouse or family member’s income is counted with theirs
- Employers who do not have a Singpass and so cannot use MOM’s online application
- Sponsors and joint applicants, who complete and sign their own sections and declarations
How to fill out FDW Sponsorship Application
- Check that you, your sponsor and any joint applicant are all Singapore citizens or permanent residents, and that the sponsor is a child, parent or sibling as MOM requires.
- Fill in the form in soft copy: the helper’s particulars, salary, rest days and expected job scope, then the employer, household and sponsor sections.
- Gather your supporting documents in PDF or JPG, including proof of income such as IRAS Notices of Assessment or CPF statements, and the personal particulars page of the helper’s passport. Any document not in English needs a translation.
- Sign the declarations, including the confirmation that you are the employer and not applying through an employment agency.
- Upload the completed form and documents and pay the application fee through the Ministry of Manpower online form linked on the last page. Payment can be made by PayNow, Visa, Mastercard or Amex.

Frequently asked questions
Can an employment agency submit this form for me?
No. The paper form states that employment agents should not use it. Agencies apply through MOM’s FDW eService with their employment agency account instead.
