What is Bereavement Care Form?

When an Optus customer passes away, someone acting for the estate uses this form to tell Optus and to request what happens next, such as closing the account, cancelling or transferring services, or taking over a mobile or internet service. It collects the details of the deceased customer and their account, the details of the person making the request and their relationship or authority to act, and instructions for each service on the account.

Optus asks for supporting documents to verify the death and the requester’s authority, which typically include a death certificate and, where relevant, proof that you are the executor or authorised representative of the estate. Providing these with the form helps Optus act on your instructions without repeated requests. The form is a standard fillable PDF, so you can complete it on screen before signing.
