What is N/A?
This is the standard NFS@Gov direct credit form used across Singapore ministries, departments and statutory boards. The Singapore Courts publish it under their attending court forms so that a witness, or an organisation being paid by the courts, can register bank details once and receive payments by direct credit rather than by cheque.
Part I is completed by the ministry, department or statutory board making the payment. Part II is completed by the person or entity being paid and asks for the UEN, NRIC or FIN, address, telephone, a mandatory email address for payment notifications, GST registration status, the account holder name, and the bank, branch and account numbers. Part III is the bank's certification that the signature matches its records.

How to fill out N/A
- Print the blank form; the courts accept originals only, and correction tape or fluid must not be used anywhere on it.
- Complete Part II in full, using your name exactly as it appears on your NRIC or FIN, or your ACRA registered name for an organisation.
- Enter the bank, branch and account numbers, add the mandatory email address for payment notifications, and sign as your bank has on record.
- Get your bank to certify Part III, unless you are an ACRA registered organisation, Singapore Citizen or Permanent Resident banking with DBS, POSB, OCBC, UOB, Far Eastern Bank or Citibank, in which case Part III is left blank.
- Mail the original form, not a fax or scan, to the ministry, department or statutory board you are dealing with, which for witness expenses is the court handling your case.

Frequently asked questions
Can I submit a scanned copy?
No. The form states that only originals are accepted and that a fax copy is not acceptable, so the signed paper form has to be posted or delivered.
When do I need my bank to sign Part III?
Part III can be left blank if you are an ACRA registered organisation, a Singapore Citizen or a Permanent Resident and you hold an account with DBS, POSB, OCBC, UOB, Far Eastern Bank or Citibank. Everyone else needs the bank to certify the signature.
Why is the email address compulsory?
The form says payment notification is sent to the email address you give, so an incomplete or missing email means you will not be told when a payment has been credited.
