What is ETS-1?
ETS stands for E-Template Statement. It is the structured statement the Family Justice Courts ask a complainant to file in a fresh maintenance application under section 69 of the Women's Charter, so that the court and the respondent see the same set of facts in the same order from the start.
The 16 page form runs from Part A (personal particulars and any medical condition or incapacity) through Part B (marriage and children, including whether the marriage is a Muslim marriage and whether divorce proceedings are on foot), Part C (who maintenance is claimed for and how much), Parts D1 to D3 (income, assets, debts and liabilities) and Part E (anything else the court should know). Each part names the supporting documents you must upload on iFAMS alongside it, such as payslips, CPF statements and IRAS Notices of Assessment.
Who files ETS-1?
- A wife applying for maintenance from her husband under section 69 of the Women's Charter
- An incapacitated husband claiming maintenance, supported by a medical report meeting Rule 87A of the Family Justice Rules
- A parent or guardian claiming maintenance for a child, including a claim continuing past age 21 in the circumstances the form lists
- Complainants preparing their financial disclosure before the first maintenance mediation or court session
How to fill out ETS-1
- Complete Part A with your full name, NRIC or passport number and the email address the court will use for notifications, and answer the question about any disability or incapacity.
- Complete Part B with details of the marriage and each child, and say whether there are existing divorce proceedings in the Family Court or Syariah Court.
- Set out the claim in Part C: whether maintenance is for you, the children or both, the amount sought and, for a child over 21, the special circumstances relied on.
- Fill in Parts D1 to D3 in full (income, assets, debts) and Part E, then gather the supporting documents each part lists, including payslips, CPF statements and IRAS Notices of Assessment.
- File the completed statement and upload the supporting documents through iFAMS for the Family Justice Courts, using the document labels named in each part.

Frequently asked questions
What is the difference between the complaint and the ETS-1?
The maintenance application itself is started through iFAMS. The ETS-1 is the statement that goes with it, setting out your circumstances, your claim and your financial position in the court's standard format so the case can be assessed and mediated.
Do I have to attach documents?
Yes. Each part of the form has a confirmation box listing the documents that belong with it, such as employment proof, payslips for the latest 3 to 6 months, CPF statements and IRAS Notices of Assessment. Check the Document and Evidence Checklist (s69) published with the form.
Can I apply if my marriage is a Muslim marriage?
The form asks whether the marriage is a Muslim marriage and whether any proceedings are before the Syariah Court, because that affects which court deals with what. Answer honestly and take advice if proceedings are already running elsewhere.
