What is T4A?
The T4A slip, Statement of Pension, Retirement, Annuity, and Other Income, covers a wide range of payments that are not regular employment earnings, including pension and superannuation, annuities, lump-sum payments, self-employed commissions, fees for services, research grants and scholarships. The payer prepares the slip, files the slip information with the CRA and gives a copy to the recipient, who reports the amounts on their income tax and benefit return.
The T4A slip is a dynamic PDF that the CRA publishes in Adobe XFA format. It does not fill in a web browser, so it will not display in the in-browser editor or quick-fill form here. Download it and open it in Adobe Acrobat Reader to complete the fields.

How is the T4A different from a T4?
A T4 reports employment income and payroll deductions from an employer. A T4A reports other income such as pensions, annuities, fees for services, commissions and scholarships from a payer.
Who issues the T4A slip?
The payer issues it, not the recipient. The payer files the slip with the CRA and gives a copy to the person who received the payment.
