What is DA Form 3749?
DA Form 3749, Equipment Receipt, is a receipt card used for the recurring issue of an item that is stored centrally but assigned to an individual, most commonly weapons kept in the unit arms room. The soldier exchanges the card for the item at issue and gets the card back at turn-in, giving the armorer a simple physical record of who holds what.
The current edition on Army Publishing Directorate is dated December 2023, with the Army G-4 as proponent. Procedures for using equipment receipts, including their preparation and control, are part of the property accountability procedures in DA Pam 710-2-1.
Who files DA Form 3749?
- Soldiers assigned a weapon or other centrally stored item they draw repeatedly

How to fill out DA Form 3749
- Enter the individual's name, rank, and unit on the card.
- Record the item description, serial number, and any components the receipt covers.
- Have the authorizing official sign the card to validate it.
- Exchange the card for the item at each issue; the armorer holds the card while the item is out.
- Return the item to get the card back, and turn the card in to the unit when you clear or the assignment changes.

Is DA Form 3749 a hand receipt?
It works alongside hand receipt procedures but is a different tool. A hand receipt documents ongoing responsibility for property; the equipment receipt card covers the repeated short term issue of an assigned item, like drawing your weapon for training and turning it back in the same day.
What happens if I lose my weapons card?
Report it to your armorer and supply sergeant right away. The unit controls issued cards, and a lost card must be accounted for and replaced under the unit's procedures before you can draw the item normally again.
