What is DA Form 1687?
DA Form 1687, Notice of Delegation of Authority - Receipt for Supplies, documents a responsible officer's delegation of authority to named individuals to request and receipt for supplies. Supporting supply activities keep the card on file and check signatures against it before issuing.
The current edition on Army Publishing Directorate is dated December 2023, with the Army G-4 as proponent. The single page form lists the unit and account information, the individuals authorized, what they may sign for, their signatures and initials, and the delegating official's authentication.

- Have each listed individual sign and initial the form so their signature is on file.
- Complete the delegation statement and have the responsible officer sign and date it.
- File the completed card with the supporting supply activity and keep a copy in unit records; update it whenever authorized personnel change.
