DA Form 2404 is the worksheet crews and mechanics use during preventive maintenance checks and services (PMCS) and inspections to record equipment faults and the corrective actions taken.

What is DA Form 2404?

DA Form 2404, Equipment Inspection and Maintenance Worksheet, records the results of equipment inspections, PMCS, and maintenance services. Each fault found is listed with its status symbol, and the corrective action column shows what was done or deferred, making the form a working record of the equipment's condition.

The current edition on Army Publishing Directorate is dated February 2011, with the Army G-4 as proponent. Use of the form is governed by the Army maintenance management system described in DA Pam 750-8. Many units record the same information electronically, but the paper worksheet is still used where automated systems are not available.

How to fill out DA Form 2404

Fill in the header with the equipment nomenclature, registration or serial number, and the applicable technical manual.
Perform the inspection or PMCS using the TM checklist for the equipment.
List each fault or deficiency on its own line with the correct status symbol.
Record the corrective action taken for each fault, or note it for deferred maintenance.
Have the appropriate maintenance supervisor review and initial the worksheet, then turn it in to the motor pool or maintenance office per unit procedure.
