What is DA Form 348?

DA Form 348, Equipment Operator's Qualification Record (Except Aircraft), documents a soldier or civilian operator's qualification history for Army vehicles and equipment other than aircraft. It records the equipment the operator is licensed on, road tests and written tests, license actions, and awards or suspensions, and it supports issuing the operator's permit.

The current edition on Army Publishing Directorate is dated March 2025, with the Army G-3/5/7 as proponent. The Army driver and operator standardization program that governs testing, licensing, and this record is in AR 600-55.

How to fill out DA Form 348
